Gender

Practitioners tell Carisk their gender when they join the network and every 3 years thereafter when they complete the recredentialing process.

Discipline / Provider type

Discipline/provider type means the practitioner’s area of professional training such as physician, psychologist, social worker, mental health counselor.

 

Practitioners tell Carisk their discipline/provider type when they join the network and every 3 years thereafter when they complete the recredentialing process. Carisk verifies discipline/provider type when practitioners join the network.

Changes to information between 3 year cycles may not be reflected until the next 3 year reporting cycle. If Carisk receives new information between cycles, the information is verified and the directory is updated.

Specialty

Specialty means the practitioner has completed additional education and training in a specific area after completing medical or professional school.

 

Practitioners tell Carisk specialty information when they join the network and every 3 years thereafter when they complete the recredentialing process. Carisk verifies specialty training at these times.

Changes to information between 3 year cycles may not be reflected until the next 3 year cycle. If Carisk receives new information between cycles, the directory is updated.

Organization Affiliations

Organization Affiliation means the provider with which the practitioner works or is associated.

 

Practitioners tell Carisk their organization affiliation when they join the network and every 3 years thereafter when they complete the recredentialing process.

Changes to information between 3 year cycles may not be reflected until the next 3 year cycle. If Carisk receives new information between cycles, the directory is updated. Carisk does not verify organization affiliation.

Board Certification

Board Certification means the practitioner has completed additional education and training in a specific area after completing medical or professional school and has successfully passed an examination indicating expertise in this area.

 

Practitioners tell Carisk board certification information when they join the network and every 3 years thereafter when they complete the recredentialing process. Carisk verifies board certification from the issuing entity at these times.

Changes to information between 3 year cycles may not be reflected until the next 3 year cycle. If Carisk receives new information between cycles, the directory is updated. Accepting New Patients

Accepting New Patients means the practitioner will see new patients in the practice.

 

Practitioners tell Carisk information about accepting new patients when they join the network and every 3 years thereafter when they complete the recredentialing process.

Changes to information between 3 year cycles may not be reflected until the next 3 year cycle. If Carisk receives new information between cycles, the directory is updated. At least annually, Carisk completes an evaluation if the practitioner is accepting new patients.

Languages spoken by the practitioner and, if applicable, the staff

Languages spoken by the practitioner or the staff means the practitioner or staff can speak in the listed language.

 

Practitioners tell Carisk information about languages spoken by the practitioner or the staff when they join the network and every 3 years thereafter when they complete the recredentialing process.

Office Locations and Phone Numbers

Practitioners tell Carisk their office location and phone numbers when they join the network and every 3 years thereafter when they complete the recredentialing process.
